Retrieve a declaration from the organ donation register


If you have submitted a declaration for organ and tissue donation in the organ donation register, you can retrieve it using your declaration ID.

The organ donation register is a central electronic register in which you can enter your decision for or against organ and tissue donation. Authorized institutions such as removal hospitals can access your declaration if it needs to be clarified whether you wish to donate your organs or tissue.

If you have already submitted your declaration, you can retrieve it from the organ donation register. To do this, you will need the declaration ID that you were able to download as a PDF after your initial declaration and received by e-mail. The declaration ID is a seven-digit combination of numbers and characters. After retrieving your declaration, you have the option of changing or deleting it.

Requirements

  • You have submitted a declaration for organ and tissue donation in the organ donation register.
  • You have access to your declaration ID, which you received after submitting your declaration.

What documents do I need?

  • eID means of identification
    • ID card with online ID function or
    • electronic residence permit or
    • eID card for citizens of the European Union and the European Economic Area
