Name: Declaration of name for children - change of name in case of later joint custody

If joint custody is not established until the child already has a name, the child's name may be redetermined within 3 months of the establishment of parental custody.

Requirements

Subsequent establishment of joint custody (at the youth welfare office or notary)

What documents do I need?

• Current extract from the birth register of the child
• custody certificate from the youth welfare office
• if necessary, a certified copy from the family register
• Proof of the other parent's name

• Consent of the child, if he/she has reached the age of 5
• Identity card or passport of the person making the declaration
