Extend application for a severely disabled person's pass


If you have a severely disabled person's ID card and its validity is about to expire or has already expired, you must apply to extend the validity period of your severely disabled person's ID card.

To extend the validity of a temporary severely disabled person's ID card, an application must be made to extend the card.

A photograph must be sent to extend the validity. An extension on the old ID card is no longer possible!

The previous severely disabled pass must be returned to the AVIB when the new pass is issued.

Requirements

If no significant change in health circumstances is to be expected, the permit can be issued or extended for an indefinite period.

What documents do I need?

  • in case of representation with written power of attorney

    Additionally: identity card or passport of the authorized person

  • Passport photo

    The passport photo can also be scanned and sent by email.
